This study used a qualitative method with phenomenological approach. Retrieving data using in-depth interview on the four participants who become case managers in Banjarmasin hospital. There are five themes found in this research that the implementation of the assessment in case management.
The first theme, the implementation of the assessment in the Case Management consists of two sub-themes, namely: (1) Step Assessment (2) Method of Assessment (3)Manner assesment. The second theme, the realization of the case management consists of three sub-themes, namely: (1) Implementation case management (2) coordination case management (3)Evaluation case management. The third theme, obstacle of case management consists of three Sub-themes, namely: (1) problemof communication(2) Facilities (3) Human Resources. The fourth theme is support of case management barriers consist of three sub-themes, namely (1) hospital policy (2)Financial (4) Environtment support.
The results of this study showed a picture implementation of Case Management by head space. Based on the findings of researchers, the head space that performed the role of Case Management of four participants, mostly in performing the role of head space with five management functions already run up to, but in the role as a Case Manager Case Management is not based on the principle intact.
